Medical assessment form in case of a health problem or disability. 6.Family photo that includes all family members. *For children under the age of 16, the supporting document of the obligation to submit the solemn declarations by both parents or the legally exercising parental responsibility for the publication of personal data is necessary. The controller of the operator shall make all reasonable efforts to establish the age and verify the identity of the person giving consent in the case of a child under 16 years of age. For children over the age of 16, the supporting document of their consent is required for any publication of their personal data. The controller of the institution must ensure that the child has been informed and understands what he/she consents to, otherwise the consent is invalid, as the obligation to provide clear information has not been met.
